Le sel : un enjeu de santé publique ?

Le sel est toujours l’objet de polémiques. Déjà, en janvier 2002, l’AFSSA (1) a rendu public un rapport sur le sel et la santé: il comportait un certain nombre de recommandations et avait pour objectif la réduction de la consommation de sel en France. Un an aprés, l’Agence a souhaité faire le point avec les industriels, les pouvoirs publics, les médecins et diététiciens et les associations de consommateurs, sur l’application de ces recommandations au niveau: - des industries agro-alimentaires - de la distribution - de la restauration collective - de la réglementation - de la sensibilisation des consommateurs. La consommation de sel a été évaluée, et une mise à jour des tables de composition des aliments a été établie. L'ANIA (2) et plusieurs de ses partenaires ont donc présenté, lors d'uneréunion de bilan organisée par l'AFSSA, un point sur les initiatives en cours en vue d'une diminution de la consommation de sel. Les actions présentées s'inscrivent bien dans la continuité de la baisse des teneurs en sel des aliments: c'est un bilan "très positif", selon l'AFSSA Mais revenons sur l'historique de cette décision de l'AFSSA. Le sel, les besoins et sa consommation Le sel est un produit naturel, apprécié et recherché par l'Homme; Plutarque disait “le sel transforme une nourriture nécessaire en nourriture agréable”. Le minimum vital se situe à 0,5 g par jour et une alimentation équilibrée sans ajout extérieur de sel assure ce besoin. Pour se donner une marge de sécurité, on considère que l'apport physiologique est d’environ 2g. La consommation moyenne actuelle des Français est très supèrieure à ce besoin: les résultats d'une enquête réalisée en 1998/1999, montrent une consommation moyenne de 8 g/jour hors sel ajouté à table et utilisé pour la cuisson des aliments, soit une estimation totale de 10 g/jour. Le goût du sel est une des sensations gustatives au même titre que le sucré, l’acide, et l’amer. Il est admis qu’un goût spécifique dû au sel est apparu au fil des années afin de stimuler la consommation de sodium. Dans notre alimentation, le sel est non seulement un exhausteur de goût, mais c’est aussi un puissant inhibiteur des arrière-goûts amers. Pour cette raison, on l’utilise très largement dans de nombreux produits alimentaires. Entre 1994 et 1999, l'augmentation de la consommation moyenne du sel de 10 à 15 % s’explique uniquement par les produits transformés ( 80 % du sel ingéré provient de ces aliments): le sel permet de fixer plus d’eau dans certains produits, de stimuler la sensation de soif et de masquer l’absence de goût. Par ailleurs, le sel a un rôle dans la conservation grâce à son effet bactériostatique. Beaucoup de qualités pour notre bon vieux sel de cuisine! le problème posé par sa consommation ne réside pas dans sa nature mais dans l’excès des quantités aujourd’hui ingérées. Les opposants à la diminution de la consommation jouent sur cette ambiguité et rappellent qu’un plat sans sel est peu gastronomique. Or, toutes les politiques de santé publique à travers le monde visent à réduire modérément la consommation non pas à la supprimer. Le sel et la santé Que reproche-t-on au sel? L'abondance des offres alimentaires conduit nos sociétés à des surconsommations à l'origine d'une nouvelle épidémie, 1ère cause de mortalité, l'obésité! Le chiffre de 25 000 morts supplémentaires liés à la consommation du sel a été avancé. Quantifier une mortalité due à des maladies multifactorielles comme l’hypertension peut apparaitre effrayant mais cela permet, peut-être, une prise de conscience. Tous les mécanismes conduisant aux pathologies liées au sel ne sont pas encore tous élucidés. Des débats entre spécialistes sont toujours d'actualité, mais une majorité s’entend pour souligner plusieurs risques.
